Professional Tax Declaration

What the employer needs from the employee before deducting professional tax: the state whose schedule applies, the monthly gross wage, whether the employee is in simultaneous employment elsewhere, and any exemption claimed. This form does not compute a slab — the rate is the one in force in that state.
